Check out this training video: Many of our clients, because of their behavior severity, need two-on-one support. This is true of this client, who was scheduled for a routine dental visit. Our task was to get the examination without harming the client or damaging the clinic equipment. Dropping to the floor is a typical response when in demand situations.
Many developmentally disabled clients have poor hygiene and have never been to a dentist. In many cases, the use of candy as a reinforcer results in a consistent sugar burden in the mouth. Once the calculous is debrided, we often find badly decayed teeth. Repairs and extractions are done with intravenous sedation.
This resident has a history of hitting his head against walls or smashing through car windows or plate glass storefront windows. This examination was completed with the support of his behavior trainer and weeks of tolerance training to achieve this level of comfort in a dental chair.
Before his dental examination, we needed to obtain a pano of his mouth. His training on this equipment took a month, and he was taught to bite a bar and tolerate the temple reminders. In addition, we had to teach him to tolerate the X-ray head moving around his head. Before training, he was more likely going to grab the rotating head and tear it apart or bang his head against the equipment, which would both injure him and damage our equipment. This procedure and his examination were completed without sedation.
